Home habilitation Services are provided, centered, and driven by each members’ needs and desired supports. The services are provided in the person’s home and community. Our team works with the members and the program to maximize the members’ independence in activities of daily living, socialization, safety and personal health, communication, and decision-making. Many of our members who utilize Home Habilitation often struggle with mental health issues. Using a team approach our professional service teams work cooperatively with families, MCOs, and other providers to create an integrated approach to providing quality home habilitation services.
